I took another look at the picture and first, I don't think that is Colin Farrell in the background. The guy with the hat, right? He has a camera in his hand, so more likely a Pap.
As for Ioan's eye, it's not big enough to tell, but I would guess contact irrtation is a more likely culprit. Plus everyone has one eye that is slightly smaller or opens less widely than the other and I've noticed that it is the case with Ioan's left eye even before the accident. Go back and look at some pictures. Those differences can also become more pronounced as we get a little older. Anyway, take a look at natural pictures not posed portraits and see what you think,
